Change of Address Procedures

Please follow the information below to make changes to your address or to your name with Human Resources, Central Payroll Bureau (CPB) and Health Benefits. 

If you work in a Circuit or District Court, send the original forms to your local HR Liaison
For AOC and District Court Headquarters, please send the forms to Human Resources.

  • Change of Address
  • An employee who recently moved must follow the steps below to change his or her address:

    • Update address in CONNECT. More detailed instructions can be found in the UPK:
      • Login to CONNECT
      • Click on the My HR tab
      • Go to Employee Self Service Links
      • Click on the Personal Information Summary
      • Click on Change Home/Mailing Address
      • Click on the Edit button
      • Edit current home address
      • Click on Save
      • Click on OK
    • Complete a new W-4 Form to change address with the Central Payroll Bureau (CPB).  There are different forms based on residency.  Please make sure to complete the correct form for the State in which you reside.  Complete a new W-4 in its entirety.
    • Complete the Health Benefits Name/Address Change form to change address with the Department of Budget and Management (DBM) Benefits Unit.
  • Name Changes
  • An employee who has recently had a legal name change must follow the steps below to change his or her name with all necessary agencies. More detailed instructions can be found in the UPK:

    • Update name in CONNECT:
      • Login to CONNECT
      • Click on the My HR tab
      • Go to Employee Self Service Links
      • Click on the Personal Information Summary
      • Click on Name Change button
      • Click on the Edit Name button
      • Enter new name
      • Click on Submit
    • Note: Before the name change is finalized in CONNECT, you must submit a copy of your Social Security Card with new name and proof of legal name change:
      • Marriage - copy of your marriage license signed by the Clerk of Court
      • Divorce - copy of divorce decree
      • Name Correction - copy of the Court order
