DOMESTIC RELATIONS FORMS
Service of Process
Service of Process:  Once you have completed all the forms, you will have to file them with the court.  If you are filing a Complaint, Petition or Motion to start a new case or re-open an old case, you will have to have the other side served with a copy of all the court papers and a summons, which will be provided by the Clerk's Office. Once the service has been completed you must file a document, signed by the person who has effected service, to prove that the other party was served. The form varies depending upon the type of service. Please download the appropriate form below.
I intend to pay the Sheriff's Office to serve the other side. (You should ask the Clerk's Office to arrange for a sheriff to serve the other side at the time you file your papers).  No forms required. The Sheriff's Office will supply the necessary affidavit.
I intend to have a friend or other adult (other than myself) who is over age 18 to serve the other side with the papers. CCDR55
